*When five to twenty five team members sign up for the same fitness package, a 10% discount will be offered on each package.  When more than twenty five team members sign up for the same fitness package, a 25% discount will be offered on each package.

There are NO REFUNDS or RESCHEDULING granted for cancellations and/or missed appointments.  (*A one-time rescheduling will be granted to participants who notify the lab staff more than 48 hours before their scheduled appointment.)

2.  Pay online:  (Do not pay before staff approval.)
