How to Request Services Each Term

Print a copy of your "Student Detail Schedule" from myLane and bring it to the Disability Resources office. Your schedule will include information we need to implement your accommodations, such as a list of your courses, instructors' names, and classroom numbers.

Number your classes on the "Student Detail Schedule" (1, 2, 3, etc.).

Transfer the numbers assigned to each class to your Accommodations Request Form. Pick up one of these forms each term from the Disability Resources office.

Turn in your Accommodation Request Form and schedule to the Front Desk in the Disability Resources office.

Contact Disability Resources staff for assistance if you would like help with completing this process.
- Finally, review your Letter of Accommodation (LOA) with each instructor (and with the computer lab staff, if necessary). To do this, meet with your instructor outside of class, during office hours or by appointment, to discuss your need for accommodations. During this discussion, review the LOA with your instructor.
Note: In the past we have used Faculty Acknowledgment Forms. These forms are now optional and will be phased out in the near future.
