Confidentiality

All medical records and information are confidential and are released only upon written permission of the patient, upon court order or as provided by law (Oregon Revised Statutes 192.525-530).

Patient Access to Records

Patients may review their own records.  An appointment to do this may be made through any Student Health Services staff member.
