Course Title: Term:___________
This student is enrolled in your class and has registered with Disability Resources with a disability that is recognized under Section 504 of the Rehabilitation Act of 1973 and the Americans with Disabilities Act of 1990. I have determined that this student is eligible for the academic accommodations identified below.

Please be discreet when discussing this or any student’s disability issues in order to protect their privacy. It is the student’s choice whether or not to share his or her confidential disability information with you or anyone else.
Thank you for your assistance with this matter. If you have any questions, problems, or concerns, please contact our office staff in Center 217 or call (541) 463-5150 or E-Mail us at DisabilityResources@lanecc.edu .

COMPUTER-ASSISTED NOTETAKER: This service may be needed when a sign language interpreter is not appropriate. The student may need to be seated next to the C-A Notetaker in order to read the monitor as the discussion/presentation/film narration is being typed.
CURRICULUM/ACADEMIC ADJUSTMENTS: written and oral directions from instructor; early notification or additional time to complete large assignments if not on syllabus; additional time to complete in-class assignments; spelling not graded; other:
FREQUENT MOVEMENT OR BREAKS: Frequent changes in sitting/standing positions; periodic breaks outside of classroom.
INTERPRETER: An interpreter may attend your class with a student who is hearing impaired or deaf. Here are some suggestions to create an accessible learning environment for those students: Speak directly to and look at the class when presenting your material. This will assist students who are speech-reading, or who need your voice to be projected toward them. When questions are asked by classmates, always restate the question for the whole class. During discussions, ask students to speak one at a time so that the interpreter can sign all comments, allowing all students to "hear" everything that is shared.
NOTETAKING (OR COPY OF INSTRUCTOR’S LECTURE NOTES): Please help the student find a competent classmate who will share notes. The student can use NCR paper (provided by DR) or arrange to have their notes copied through our office after each class. Suggested statement: "We have a need for a volunteer notetaker who will share notes with another student. Please notify me at the break if you are available for this." Student should be present to receive notes unless other arrangements have been made through our office.
TAPE RECORDER FOR LECTURE OR LAB: This student may need to use a tape recorder in lecture or lab. Taped material is to be used only for successful completion of a course and is not for duplication. The student has been instructed to discuss this with you.
TAPED, ON DISK, OR BRAILLED TEXTBOOKS/WRITTEN MATERIALS: Your assistance may be needed by our staff if materials are to be taped or printed in alternative formats.
TEST/QUIZ ACCOMMODATIONS: Extended time (1½, 2 times); distraction-free room; breaks; reader; scribe or tape record answers or word processor; music through earphones; spelling not graded; calculators and/or multiplication table; multiple choice format; other:
